I would like to learn some Polish birthday wishes and surprise my girlfriend. Can you give me a few simple sentences, please?

I just copied this from Duolingo: the most basic birthday wishes are Wszystkiego najlepszego! (All the best) and Sto lat! (100 years!).

As Roger mentioned, the standard one is:

Wszystkiego najlepszego! or Wszystkiego najlepszego w dniu urodzin! All the best (on your birthday)!

Another one could be:

Życzę ci zdrowia, szczęścia, pomyślności. I wish you health, happiness and prosperity.
